The Discharge Package

When a patient is discharged from a hospital, SNF, or rehab facility, the hours between discharge and arrival home are the highest-risk window in their recovery. Vanguard’s Discharge Package closes that window.

Memory care training is our minimum standard — not an elective.
Maryland is home to hundreds of thousands of adults living with dementia and Alzheimer’s disease — and across every county we serve, families are navigating this diagnosis without the specialized support they need. Most home care agencies send aides with no dementia-specific training. Vanguard does things differently.
Our founder holds national dementia navigation credentials, and dementia and memory care training is a required component of the Vanguard caregiver standard. Select caregivers hold Alzheimer’s Association essentiALZ® certification, the leading credential for direct care workers in dementia-specific care.
