The Care Readiness Review is a free educational tool published by Vanguard Care Solutions as part of the Care Without Crisis™ initiative. Its sole purpose is to help families think through common questions related to aging and caregiving. It is general information and nothing more.
Nothing produced by this tool is medical advice, a medical opinion, a diagnosis, a screening, a clinical assessment, or a recommendation for or against any treatment, medication, provider, or course of care. No physician–patient or clinician–patient relationship is created by using it. Always seek the advice of a licensed physician or other qualified health professional with any question regarding a medical condition. Never disregard professional medical advice or delay seeking it because of something you read here.
References to powers of attorney, advance directives, MOLST forms, wills, HIPAA authorizations, guardianship, surrogate decision-making, Medicaid eligibility, or any other legal topic are general descriptions provided for education only. No one reviews your answers. No one is monitoring this tool or standing by to respond to it.
The questions, readiness ratings, planning stage, risk indicator, planning horizon, and recommended steps are illustrative educational outputs generated by fixed logic from the answers you select. They are not a professional assessment, care plan, clinical evaluation, prognosis, or prediction about any individual. Ratings and scores are relative educational indicators only and carry no clinical, legal, or financial meaning. Two families with similar answers may face entirely different realities.
This tool is provided as a convenience and may contain errors, omissions, or information that has become outdated. Its content is general and cannot account for individual medical, legal, financial, or family circumstances. It is not exhaustive; important considerations relevant to your situation may not appear here at all.

You use this tool entirely at your own risk and remain solely responsible for your own decisions.
Your answers. The answers you select while using this tool are processed entirely within your own browser. They are not stored on any server, no account is created, and they are discarded when you close the page. Nothing is required to view, print, or save your full results.
If you choose to give us your email address. At the end of the tool you may optionally ask us to email you a copy of your plan, ask to be reminded to revisit it in six months, ask to receive our educational emails, or ask a care advisor to contact you. Each of those is a separate, voluntary choice, and none is required.
If you make one of those choices, we receive the contact details you enter together with a summary of your results — your planning stage, overall readiness rating, category ratings, planning horizon, the first few suggested steps, and which family member you indicated you are planning for. We do not receive the individual answers you gave about anyone’s health, memory, mobility, medications, finances, or daily needs. Those remain in your browser and are never transmitted.
Contact details you provide are stored in our customer relationship management system and used to send what you asked for and, if you opted in, our educational emails. We do not sell, rent, or trade your information, and we do not share it with third parties for their own marketing. Every marketing email includes a one-click unsubscribe, and you may ask us at any time to delete your information by emailing info@vanguardcaresolutions.com. If you asked only that we email your plan, we will not add you to any marketing list.
This is not a HIPAA-covered service. In connection with this tool, Vanguard Care Solutions is not acting as a HIPAA covered entity or business associate, and information submitted here is not protected health information. Please do not enter medical record numbers, insurance identifiers, Social Security numbers, or other sensitive identifiers anywhere in this tool. If you print or save a copy of your plan, safeguarding that copy is your responsibility.
Any reference to a government agency, program, professional association, service, or organization is provided for informational purposes only and does not constitute an endorsement, recommendation, or guarantee. Vanguard Care Solutions is not responsible for the content, availability, accuracy, or practices of any third party.
Care Without Crisis™ is an educational and advocacy initiative presented by Vanguard Care Solutions, a licensed Maryland Residential Service Agency. This tool contains no promotion of Vanguard services, and any mention of Vanguard is informational only. Using this tool creates no obligation of any kind and does not enroll you in anything.
